Visual Signs of Lipodystrophy

Dark skin patches (acanthosis nigricans)
Dark, velvety, thickened skin that appears in body folds such as the neck, armpits, and groin, linked to insulin resistance.

Unusual fat accumulation
Excess fat building up in unexpected places such as the face, neck, or between the shoulder blades (sometimes called a “buffalo hump”).

Muscular appearance & visible veins
Unusually defined or prominent muscles, especially in the arms and legs, even without exercise, due to the absence of fat. Veins appear prominently under the skin because there is little fat to cover them.

Fatty skin bumps (xanthomas)
Itchy or painful yellowish deposits that form in the skin or near joints when blood fat levels are very high.
Quality of Life Impacts
Extreme hunger (hyperphagia)
Constant, overwhelming hunger that is difficult to manage even after meals.
Fatigue
Persistent tiredness, even after a full night’s rest.
Pain
Chronic muscle and joint pain impacting daily mobility.
Social anxiety

Body dysmorphia & depression

BRAIN – Mental & Cognitive Effects
What Is It?
The brain is the body’s command center, controlling thoughts, memory, emotions, and every process that regulates the body. In lipodystrophy, the brain is affected both directly — through metabolic disruption — and indirectly through the psychological burden of living with a rare, visible condition.
How Does It Work?
Lipodystrophy disrupts leptin and insulin signaling, which directly affects brain circuits that regulate hunger, mood, and cognition. The hypothalamus, which controls appetite, can be thrown into overdrive, causing extreme hunger. Metabolic imbalances also impair neurotransmitter function, contributing to depression, anxiety, and brain fog.
Extreme hunger (hyperphagia)
A constant, overwhelming sense of hunger that is very difficult to satisfy, even after eating.
Body image distress (body dysmorphia)
Persistent distress or preoccupation with the way the body looks, often related to visible fat changes.
Depression
Persistent feelings of sadness, hopelessness, or loss of interest in daily life.
Anxiety
Ongoing worry, nervousness, or fear that can interfere with everyday activities.
Social anxiety
Self-consciousness and discomfort in social situations due to visible changes in body appearance.
Emotional burden
The stress of living with a rare, often misunderstood condition can take a significant toll on mental health and overall wellbeing.
Brain fog and memory problems (reported by patients)
Difficulty concentrating, thinking clearly, or remembering things.
Headaches (reported by patients)
Frequent or chronic head pain.
Fatigue
Persistent, debilitating tiredness that doesn’t go away with rest.
AIRWAYS/THROAT – Breathing & Swallowing
What Is It?
The airways and throat form the passageway for air to reach the lungs and for food to reach the stomach. In lipodystrophy, abnormal fat deposits or metabolic complications can affect breathing and swallowing.
How Does It Work?
Unusual fat accumulation around the neck and throat can narrow the airway, particularly during sleep. Metabolic inflammation and tissue changes can also affect the esophagus, making swallowing difficult or uncomfortable.
Sleep apnea
Breathing that repeatedly stops and starts during sleep, causing poor sleep quality.
Esophageal problems
Difficulty swallowing or discomfort in the esophagus, the tube connecting the throat to the stomach.
HEART — Cardiovascular Complications
What Is It?
The heart is a muscular organ that pumps blood throughout the body. In lipodystrophy, the heart is at significant risk due to metabolic complications including insulin resistance, high triglycerides, and ectopic fat deposition in the heart muscle.
How Does It Work?
When fat cannot be stored properly in adipose tissue, it accumulates in organs including the heart. This ectopic fat can stiffen or weaken the heart muscle, disrupt electrical signals, and eventually lead to heart failure. Combined with atherosclerosis from high blood lipids, the cardiovascular system faces multiple threats.
Heart muscle disease (cardiomyopathy)
The heart muscle becomes weakened or stiff, making it harder to pump blood effectively.
Heart failure (congestive heart failure)
The heart can no longer pump blood efficiently, causing fluid to build up in the lungs and body.
Irregular heartbeat (arrhythmias)
The heart beats too fast, too slow, or unevenly.
BLOOD VESSELS — Vascular Complications
What Is It?
Blood vessels are the network of arteries, veins, and capillaries that carry blood throughout the body. In lipodystrophy, chronic high triglycerides and insulin resistance accelerate damage to blood vessel walls.
How Does It Work?
Elevated blood lipids cause fatty plaques to build up inside artery walls (atherosclerosis), narrowing the vessels and reducing blood flow. The metabolic stress also raises blood pressure, further straining the vessel walls. Loss of subcutaneous fat can make veins unusually visible.
Artery narrowing (atherosclerosis)
Fatty deposits build up inside artery walls, restricting blood flow and raising the risk of heart attack and stroke.
High blood pressure
The force of blood against artery walls is consistently too high, straining the heart and blood vessels.
LIVER — Hepatic Complications
What Is It?
The liver is the body’s largest internal organ, performing over 500 functions including detoxification, metabolism, and bile production. In lipodystrophy, the liver becomes a major site for ectopic fat storage, leading to progressive liver disease.
How Does It Work?
When adipose tissue is absent or dysfunctional, excess fat is redirected to the liver. This causes fatty liver disease, which can progress to inflammation (steatohepatitis), fibrosis, and eventually cirrhosis. The liver also struggles to manage the extreme triglyceride levels common in lipodystrophy.
Fatty liver disease (MASLD)
Excess fat builds up in the liver, causing inflammation and, over time, potential liver damage.
Liver scarring (fibrosis / cirrhosis)
Advanced liver damage in which healthy liver tissue is replaced by scar tissue, reducing the liver’s ability to function.
Enlarged liver (hepatomegaly)
The liver becomes abnormally large, often noticed during a physical exam or imaging.
PANCREAS — Pancreatic Emergencies
What Is It?
The pancreas produces digestive enzymes and hormones like insulin. In lipodystrophy, the pancreas is under constant stress from extreme insulin resistance and dangerously high triglyceride levels, making it vulnerable to acute inflammation.
How Does It Work?
Very high triglyceride levels — common in lipodystrophy — can trigger acute pancreatitis, a painful and potentially life-threatening inflammation of the pancreas. Repeated episodes can cause permanent damage to the organ and further impair its ability to produce insulin and digestive enzymes.
Pancreas attacks (recurrent pancreatitis)
Repeated episodes of painful inflammation of the pancreas, often triggered by very high triglyceride levels — and a medical emergency when severe.
STOMACH — Digestive Complications
What Is It?
The stomach is a muscular organ that breaks down food using acids and enzymes. In lipodystrophy, autonomic nerve dysfunction and metabolic changes can impair normal stomach motility.
How Does It Work?
Gastroparesis occurs when the stomach’s ability to contract and move food into the small intestine is impaired. This can be caused by autonomic neuropathy or metabolic disruption, both of which are associated with lipodystrophy. Food sits in the stomach too long, causing bloating, nausea, and discomfort.
Delayed stomach emptying (gastroparesis)
The stomach empties too slowly, causing bloating, nausea, and discomfort after eating.
KIDNEYS — Renal Complications
What Is It?
The kidneys filter blood, remove waste, and balance fluids and electrolytes. In lipodystrophy, chronic metabolic stress from insulin resistance, high blood pressure, and inflammation progressively damages the kidneys.
How Does It Work?
Insulin resistance and high blood pressure damage the tiny filtering units (nephrons) in the kidneys. Over time, the kidneys begin leaking protein into the urine and lose their filtering capacity. If unchecked, this progresses from inflammation to chronic kidney disease and potentially kidney failure.

The kidneys are leaking protein into the urine, a sign they are not filtering blood properly.
Kidney inflammation (glomerulonephritis)
Inflammation of the tiny filters in the kidneys, impairing their function.
Chronic kidney disease
Gradual, long-term loss of kidney function due to metabolic complications.
Kidney failure
The end stage of chronic kidney disease, where the kidneys can no longer function on their own and dialysis or transplant may be needed.
REPRODUCTIVE ORGANS — Reproductive Complications
What Is It?
The reproductive organs are key to fertility and hormonal balance. In lipodystrophy, hormonal imbalances caused by insulin resistance and altered fat-hormone signaling can significantly disrupt reproductive health.
How Does It Work?
Adipose tissue plays a critical role in producing and regulating reproductive hormones. When this tissue is absent or dysfunctional, it can lead to excess androgen production, irregular ovulation, and hormonal imbalances that affect fertility and menstrual cycles.
Polyendocrine Metabolic Ovarian Syndrome (PMOS)
A hormonal condition affecting the ovaries that can cause irregular periods, cysts, and fertility problems.
Excess male hormones (hyperandrogenism)
Higher-than-normal levels of androgens in women, which can cause symptoms like excess hair growth and irregular periods.
Irregular periods
Menstrual cycles that are unpredictable, absent, or abnormal due to disrupted hormone balance.
Difficulty getting pregnant (infertility)
Challenges conceiving.
MUSCLES — Muscular Effects
What Is It?
The muscular system enables movement, posture, and organ function. In lipodystrophy, the absence of subcutaneous fat gives muscles an unusually prominent appearance, and metabolic complications can cause weakness and pain.
How Does It Work?
Without the normal layer of subcutaneous fat, muscles become very visible and may appear unusually defined. Metabolic myopathy — muscle damage from metabolic dysfunction — can cause progressive weakness. Chronic inflammation also contributes to muscle pain.
Fun Fact: The human body has over 600 muscles. In lipodystrophy, the lack of overlying fat can make even small muscles appear highly defined — a feature that is often misinterpreted as athletic fitness.
Muscle weakness (muscular dystrophy)
Progressive weakening of the muscles over time.
Muscle pain (myalgia)
Aching or soreness in the muscles.
BONES & JOINTS — Skeletal Complications
What Is It?
The skeletal system provides structure, protection, and movement. In lipodystrophy, metabolic imbalances and the loss of fat cushioning lead to bone weakening, deformities, and chronic pain.
How Does It Work?
Insulin resistance and hormonal disruption affect bone density and remodeling. Without fat to cushion joints and pressure points, everyday activities like sitting and lying down cause pain. Some subtypes are associated with bone cysts and spinal curvature.
Bone deformities
Abnormal shaping or development of bones.
Curved spine (scoliosis)
Abnormal sideways curvature of the spine.
Bone cysts
Fluid-filled pockets in the bone.
Weak bones (osteopenia / osteoporosis)
Reduced bone density that increases the risk of fractures.
Balance problems
Difficulty maintaining stability when standing, walking, or moving.
Pain from lack of cushioning
Without fat as a natural cushion, pressure on bones and joints causes pain — especially when sitting or lying down.
Widespread pain
Chronic pain throughout the body affecting daily function.
IMMUNE SYSTEM — Immune Dysfunction
What Is It?
The immune system defends the body against infections and disease. In lipodystrophy, chronic metabolic inflammation and hormonal disruption weaken immune function and can trigger autoimmune responses.
How Does It Work?
Adipose tissue plays an important role in immune regulation. When it’s absent or dysfunctional, the immune system can become both weakened and overactive — less able to fight infections while simultaneously attacking healthy tissues. Chronic low-grade inflammation throughout the body is a hallmark of lipodystrophy.
Weakened immune system
The body is less able to fight off infections and illness.
Frequent infections or slow healing
Getting sick more often than usual, or wounds and cuts that take longer to heal.
Autoimmune disorders
The immune system mistakenly attacks the body’s own healthy tissues.
Body-wide inflammation
The immune system is in a constant state of low-level activation, causing widespread discomfort.
SPLEEN — Splenomegaly & Related Issues
What Is It?
The spleen is an organ in the upper left abdomen that filters blood and supports immune function. In lipodystrophy, metabolic stress and chronic inflammation can cause it to enlarge.
How Does It Work?
Excess fat deposits in the liver and chronic inflammation place strain on the spleen, which can become enlarged (splenomegaly). An enlarged spleen may cause discomfort and impair its ability to filter blood properly.

HORMONES — Metabolic & Hormonal Disruption
What Is It?
The blood and endocrine system carry hormones, nutrients, and waste throughout the body. In lipodystrophy, these systems are severely disrupted — insulin resistance, diabetes, dangerously high blood fats, and dysfunctional leptin signaling are hallmark features of the condition.
How Does It Work?
Without functional adipose tissue, the body cannot properly regulate insulin or blood lipids. Cells become resistant to insulin, forcing the pancreas to produce more and more — eventually leading to diabetes. Meanwhile, triglycerides can reach dangerously high levels, putting organs like the pancreas, liver, and heart at risk. The lack of fat tissue also means critically low levels of leptin — a hormone that regulates hunger and metabolism.
Severe insulin resistance
The body stops responding properly to insulin, making it hard to control blood sugar levels.
Diabetes (lipoatrophic type)
A form of diabetes caused by the lack of fat tissue, often mistaken for Type 2 diabetes.
Very high blood fats (hypertriglyceridemia)
Dangerously elevated triglyceride levels in the blood, which can trigger serious complications.
Low leptin levels (hypoleptinemia)
The body produces very little leptin, a hormone made by fat cells that regulates hunger and metabolism. Note: normal leptin levels do not rule out lipodystrophy.
Leptin resistance
Even when leptin is present, the body stops responding to its signals properly (in some subtypes).
